Postnatal Recovery & Care Guide for New Mothers

By Dr. Ankita Rajesh Shah (MS OBGY, MRCOG UK) | Matunga East, Mumbai

The postpartum period — often referred to as the "fourth trimester" — is a critical phase of physical involution, hormonal recalibration, and emotional adaptation. Caring for a newborn is paramount, but prioritizing maternal recovery is equally essential.

Key Pillars of Physical Recovery

1. Uterine Involution & Lochia Flow

After birth, the uterus contracts continuously to return to its pre-pregnancy size. You will experience postpartum vaginal discharge known as Lochia:

  • Lochia Rubra (Days 1 to 4): Dark red blood flow with small clots.
  • Lochia Serosa (Days 5 to 10): Pinkish-brown discharge.
  • Lochia Alba (Weeks 2 to 6): Yellowish-white discharge fading gradually.

2. Perineal & Episiotomy Wound Care

For vaginal deliveries with episiotomy or tearing, keep the perineum clean using warm sitz baths. Pat dry gently and use supportive sanitary pads (avoid tampons during the postpartum recovery period).

3. Caesarean Section Incision Care

Keep the abdominal surgical wound clean and dry. Avoid heavy lifting (anything heavier than your baby) for the first 6 weeks to prevent incision strain or incisional hernia.

4. Pelvic Floor Rehabilitation (Kegel Exercises)

Pregnancy and delivery strain the pelvic floor muscles supporting the bladder and uterus. Gentle pelvic floor muscle contractions (Kegels) help restore urinary continence and pelvic stability.

Emotional Wellness & Postpartum Blues vs Depression

Up to 80% of new mothers experience "baby blues" — mild mood swings, tearfulness, and fatigue lasting 10 to 14 days due to rapid progesterone drop. However, if anxiety, deep sadness, or inability to bond with the baby persists beyond two weeks, it may indicate Postpartum Depression (PPD), which requires compassionate clinical support.
